If you are making a referral for yourself:

Once a referral has been received by the Partnership, we will contact you to advise you on the outcome (either acceptance of the referral or signposting to another agency.)

If you are accepted, you will be contacted to arrange an appointment to complete an assessment of need. This will enable us to offer you the most appropriate support.

You will then be advised on the outcome of the appointment.

If you are making a referral for someone else:

We will update you at every stage of the process, where we have consent to do so.
